CHC Funding Checklist

The NHS CHC Checklist is the screening tool used to decide whether someone should move forward to a full NHS Continuing Healthcare assessment.

This preparation tool helps you:

  • Understand what the Checklist looks for,

  • Work through the key areas of need,

  • Organise relevant information in advance of that screening decision.

CHC Assessment Preparation

The Decision Support Tool (DST) is used during a full NHS Continuing Healthcare assessment to consider needs across all domains and how they interact.

  • Prepare for a DST discussion with greater clarity and structure.

CHC Decision Review

After a full NHS Continuing Healthcare (CHC) assessment, a decision is made based on the Decision Support Tool (DST) and supporting rationale.

This review tool is for families who have received a CHC decision and want help understanding how that decision was reached.

​

It helps you:

  • Make sense of the DST scores, domain comments, and outcome letter

  • Understand how needs were considered and weighed within the framework

  • Identify areas where clarification or further questions may be reasonable
